Cushing's Syndrome Caused by Adrenal Gland

Abstract
|
|
|
22 patients were reviewed who under went adrenalectomy for Cushing's syndrome caused by adrenal gland during thr period from 1981 to 993 at surgical department of Kyunghee University Hospital.
1) The overall male to female sex ratio was 5:17.
2) The mean age was 33 years old and the peak incidence of age was in third and fourth decades.
3) The most frequent presenting clinical manifestation were moon face(81.8%), hypertension (80.0%) weight gain(77.2%), weakness & fatique(63.6%), in descending order.
4) The sensitivity of computed tomography in diagnosing adenal tumor was 100% and that of abdominal sonography was 82%. The smallest size of lesion was 2.5¡¿2.0cm in both study.
5) Adrenalectomy was performed transabdominally in 9 patients and through lumbar incision in 13 patients. The operation time, postoperative NPO time, postoperative hospital day were short in lumbar approached potients.
6) After adrenalectomy clinical manifestation was improved in all patients who can be followed-up except adrenal cancer. Hypertension was cured in 11 out of 12 patients. Weight gain was corrected in 15 patients.
7) corticosteroid replacement therapy was performed in all patients in all patients and the man length was 10.5 months.
8) There was only transient clinical improvement in adrenal cancer patients after adrenalectomy and all patients had recurrence and metastasis. The survival length was from 1.5 yr to 6yrs.
